Rotating weekends.JOB SUMMARY:Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual's health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. Care Coordination improves access to services to improves outcomes and reduce over-utilization of resources.MINIMUM QUALIFICATIONS:Graduate of accredited school of Nursing, BSN degree preferred. Current RN licensure by the Louisiana State Board of Nursing. Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, American Red Cross. The ILCOR course must include a manikin check off.Ability to demonstrate effective written and verbal communication skills and possess emotional andprofessional maturity. Basic understanding of Medicare, Medicaid, and Managed Care regulations.Organized and efficient in establishing priorities and performing tasks within specific timeframes.Self-motivated requiring minimal supervision. Views problems as a challenge and investigatesalternatives which demand creativity, flexibility, and the ability to work under stress. Responsible for maintaining strict confidentiality of all verbal and written information and documents. Familiarity with and appropriate use of community resources.Preferred Qualifications: 3-5 years of hospital adult acute care nursing experience preferred. Certification by Commission for Case Manager Certification as a Certified Case Manager (CCM) or Accreditation by the American Case Management Association as an Accredited Case Manager (ACM) is preferred.PHYSICAL DEMANDS:Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required.
