Job Title: RN Care Delivery Manager

Job Description

The RN Care Delivery Manager is responsible for supporting nursing standards of excellence with advanced professional education to deliver high-quality care. This role serves as a member of the management team, working alongside all departments and employees to support and oversee day-to-day operations. The manager provides guidance and best practices to achieve the organization's objectives, goals, and mission. This role also ensures care coordination that is sensitive to diverse backgrounds and conditions.

Responsibilities

  • Review and ensure effective development and execution of personalized care plans that improve satisfaction and outcomes for all participants.

  • Facilitate and complete the process of Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR), and other meetings, including completing attestations as needed. Support and collaborate with the Virtual IDT as required.

  • Serve as a manager on call during non-operational hours and follow up on necessary documentation.

  • Balance and allocate staffing and resources throughout the site to ensure adequate team assignments and seamless operation.

  • Ensure documentation for participants is complete and accurate, including enrollment, disenrollment, electronic/paper medical records, and other clinical notes.

  • Collaborate with the Transitions in Care Team to provide care management services across sites and communicate with team members, facility discharge planners, and home care coordinators during care transitions.

  • Ensure documentation complies with CLHC standards and federal/state regulations.

  • Collaborate with the Human Resources Department in conducting interviews, coaching, and disciplinary actions if needed. Assist in onboarding and professional development training of staff to ensure compliance with policies, procedures, and regulations.

  • Evaluate the work performance and competency of direct reports, providing ongoing informal feedback, counseling, and support, and completing annual performance reviews.

  • Act within the scope of the individual's authority to practice.

Essential Skills

  • Current active and unrestricted license and registration in New York State as a Registered Nurse.

  • Supervisory skills and management experience.

  • Legally authorized to practice in the state.

  • Medically cleared for communicable diseases and up-to-date immunizations.

  • Care management and case management skills.

  • Customer service experience.

Additional Skills & Qualifications

  • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency, long-term home health care, acute care, medical-surgical, critical care, nursing home, or diagnostic and treatment clinic.

  • Experience working with a frail or elderly population, or appropriate training provided upon hiring if less than one year of experience.

  • Managed long-term care insurance experience is beneficial.

  • Bilingual skills are preferred.

Work Environment

This position is located in an onsite medical clinic within a community healthcare setting. The role involves working at locations in Seneca and Westchester. The working hours are from 8:30 AM to 5:00 PM, with a weekly schedule of 37.5 hours. Pay and Benefits

The pay range for this position is $120000.00 - $125000.00/yr.
