Pay Range: $35.47 - $58.53The base pay range is a good faith representation of current potential base pay for successful applicants. Shift differentials have been included in the range. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity and contractsJob SummaryThe Care Coordinator will serve as the contact point, advocate, and informational resource for patients diagnosed with C. difficile infection. They will establish a relationship with eligible patients prior to discharge, follow-up with patients for up to 56 days to provide patient education, ensure access to medications, and act as a resource for patients should they have any signs and symptoms of recurrence . They increase comprehension through culturally and linguistically appropriate education and create and promote adherence to a care plan that is developed in coordination with the patient, primary care provider, and family/caregiver(s).Job RequirementsMinimum Education RequirementGraduate of an accredited school of nursing.Minimum Experience RequirementCare coordination and/or case management experience is desirable.Evidence of essential leadership, communication, education, and counseling skills. Proficiency in communication technologies (email, cell phone, etc.) Highly organized with ability to keep accurate notes and records. Experience with health IT systems and reports is desirableLocal knowledge about and connections to community health care and social welfare resources is desirableMust possess and demonstrate knowledge of the growth and development of varied age groups, neonatal to geriatric. Demonstrated ability to handle confidential hospital information. Well developed critical thinking skills. Knowledge and understanding of medical terminology. Ability to communicate effectively with medical professionals of all disciplines. Clear verbal and written communication skills, excellent phone etiquette. Ability to identify potential and actual patient occurrences that may be reportable incidents or those requiring further internal investigation.Minimum License RequirementCurrent NYS licensure as an RN.
