REGISTERED NURSE CARE MANAGER (RN), full time home health

PruittHealth @ Home is growing and is seeking two full time RN/Registered Nurses to support home health patients that reside throughout Gilmer and Pickens County, GA. Home health experience is not required. You bring the desire to work in a home health setting and we will supply the rest!

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a home care setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
