Acute Care Managers facilitate the care of our inpatients from admission through discharge in collaboration with the associated care team, ensuring progression of stay and efficient utilization of needed resources individualized to patient needs. In addition to navigating patients through both their expected acute stay and ongoing discharge needs, this nursing role is primarily focused on patient education and validation of patient understanding and promoting quality patient care outcomes. Strong communication and interpersonal relationship development skills are critical. This nursing role is patient-facing, responsible for collaborating and communicating discharge plans daily with the patient, family, and interdisciplinary departments.
