RN Care Manager - Atlas Search : Job Details

One of NYC's most respected community healthcare networks is currently seeking RN Care Managers to coordinate medical services for individuals with intellectual and developmental disabilities. This is mainly an administrative care management role located in an outpatient facility in the Bronx, with limited clinical responsibility. RNs with Care Management and developmental disabilities experience are encouraged to apply directly to this job post.

The base pay range for this position is $55+ per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:

• Prepare and review medication orders and certify staff's daily medication count
• Participate in consumer's placement for both pre-admission or discharge
• Supervise all medical and health care coordination
• Hospitalization - procure and disseminate information and monitor post op of consumer
• Complete staff trainings and host on-site in-service education to healthcare personnel
• Provide and access medical and nursing services for the people within the residential program
• Ensure treatment as prescribed is carried out through both staff and the nursing department
• Routinely complete house checks to ensure that staff are following each plan of care

Requirements:

• Bachelor of Science in Nursing
• NYS RN License
• Prior care management experience preferred
• Previous experience working with adults with intellectual and developmental disabilities and/or experience within Article 16 Clinics and/or group/residential homes preferred
