Job DetailsFull time RN Case Manager role Monday thru Friday 830am to 5 pmThe RN Case Manager will use clinical tools, data and other information to complete assessments of patient needsCreate individualized and comprehensive case management and treatment plansDetermine insurance eligibility for various treatments and treatment programsClearly explain the reason for prescribed treatments and treatment plans to patientsEducate patients on benefits, services and other resources, including payment optionsCommunicate with patients, physicians and insurance companies to secure successful outcomesDocument complex medical information to outline and improve health resultsFollow case and quality management processes in compliance with regulatory guidelines and company proceduresInterested in hearing more?
