Adecco Healthcare is searching for an **RN Case Manager** to become part of our client's team in **Columbia, SC** - 29203. This is a direct-hire, full-time, and permanent opportunity.
**Pay**
$39-$41 per hour
**Shift**
- Monday-Friday
- 8:30am-5pm or 8am-4:30pm
- Occasional on-call weekend may be required
**Qualifications**
_Required:_
- Associates or Bachelor's Degree in Nursing.
- 1 year of experience as an RN.
- Must have either ED, ICU, Home Health, Med-Surg, or similar experience.
- Ability to work with Drains, IV Pumps, Tubes, and give meds.
- Must have driver's license and reliable transportation.
- BLS Certification.
_Preferred:_
- RN with Associate's Degree with experience.

**Job Summary**
We are seeking a dedicated and compassionate RN Case Manager to join our hospice care team. This role is ideal for a nurse who has a genuine passion for working with hospice patients and their families. The RN Case Manager will play a vital role in providing high-quality care and support to patients in their homes or at care facilities.
_Key Responsibilities:_
- Patient Care: Provide comprehensive nursing care to hospice patients, ensuring comfort and dignity during their end-of-life journey.
- Patient and Family Education: Educate patients and their families about care plans, medications, and available resources, helping them make informed decisions about their care.
- Home Visits: Conduct regular visits to patients in their homes or at facilities, assessing their needs and adjusting care plans as necessary.
- Team Collaboration: Attend bi-weekly staff meetings to collaborate with interdisciplinary team members, sharing insights and strategies to improve patient care.
- Relationship Building: Establish trusting relationships with patients and their families, providing emotional support and encouragement throughout the hospice process.
