What will you do?

The Registered Nurse (RN) Case Manager plans, organizes and directs hospice care with emphasis on community health education/experience. You will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

* You will complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.

* You will complete, maintain, and submit accurate and relevant clinical notes regarding patient's condition and care given.

* Travel throughout the day and sometimes after-hours

* Case Managers also work an on-call schedule on a rotating basis.
