Responsibilities
+ Conduct intake assessments with patients and families to understand their needs and develop individualized treatment plans.
+ Ensure that families feel sufficiently equipped and supported throughout the treatment process.
+ Develop and adjust treatment protocols as needed, considering physical, financial, and other constraints.
+ Coordinate care plans and work with other healthcare providers to ensure that patients receive the necessary care.
+ Monitor patients' recovery progress and update care plans as necessary.
+ Maintain accurate and up-to-date patient records and documentation.
+ Provide education and support to patients and their families regarding health conditions and treatment plans.
+ Collaborate with medical staff and other team members to provide high-quality care.
+ Act as the point person and primary contact champion for the site location.
+ Complete MAT intakes by phone or in person.
+ Complete MAT patient care plans and recovery support plans.
+ Document all encounters and complete necessary consent forms to coordinate care with external agencies.
+ Complete discharge paperwork in conjunction with the MAT provider, including drafting and scanning.
+ Triage presenting issues and provide basic nursing skills during every patient visit.
+ Pre-visit planning on all charts, loading correct templates, updating consents, and checking for any open referrals.
+ Participate in opioid overdose prevention education and training for staff and patients.
+ Manage internal, external, and Health Home referrals.
+ Attend and present at weekly case conferences.
+ Provide patient education on stigma and opioid overdose prevention.
+ Coordinate with pharmacy and manage outreach, scheduling, and rescheduling of MAT appointments.
+ Monitor patient panels, including managing MAT excel spreadsheets for case conferences, discharges, and patient master lists.
+ Participate in MAT program initiatives or other duties as assigned.
Essential Skills
+ Registered Nurse License - NYS
+ Bachelor's degree in Nursing
+ CPR Certification from either the AHA, ARC, or National CPR
+ Experience in infection control and case management
+ Ability to communicate in English, both orally and written
+ Supervisory experience
+ Knowledge of nursing standards and competence in nursing skills
+ Experience with substance abuse populations
+ Strong commitment to public and community health
+ Ability to work with individuals from a variety of ethnic and socioeconomic backgrounds
Additional Skills & Qualifications
+ Understanding of harm reduction approaches
+ Empathic understanding of medical and psychosocial development needs
+ Ability to complete I-STOP & PNP in tandem with a provider
+ Experience with opioid overdose prevention and prior authorizations
+ Proficiency in handling labs, reviewing and flagging results
+ Experience in collecting urine for UDS (urine drug screens) and conducting pregnancy tests
Work Environment
Training takes about 90 days and will be fully onsite. Once training is complete, employees can work two days remotely. This role involves working closely with a multidisciplinary team, using the nursing process to assess, plan, implement, and evaluate patient care and resource use. The work environment supports community healthcare, offers great benefits upon transitioning to a permanent employee, promotes growth and education, and encourages long-term success with opportunities for promotion from within. Employees are eligible for a $2700 sign-on bonus after going permanent, with half paid after 90 days and the remaining half after 180 days. Additionally, tuition reimbursement is available once an employee has been with the organization for a year, providing 100% reimbursement up to $2,000 per year at non-union locations.
