RN Case Manager - Affirmed Home Care : Job Details

As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client's ongoing needs in a private pay, concierge level home care. This role requires 70% fieldwork for a small caseload of clients demanding great attention to detail. The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience. QUALIFICATIONS:

• Current NYS RN license in good standing
• BSN preferred
• Minimum of two years recent clinical experience within a Home Care setting
• Strong assessment and organizational skills
• Strong time management skills
• Prior supervisory experience with field staff
• Proven track record of working independently

PRIMARY RESPONSIBILITIES:

• Ensuring the highest quality of client care
• Developing care plans
• Obtaining necessary physician orders for home care cases as required by State regulations
• Reporting changes in the client's medical or mental condition to the attending physician and the Agency's team involved in the client's care
• Supervising and supporting the home care team
• Striving to improve the lives and sustainability of our clients by supporting their autonomy in the home environment
