*\*\*NEW WAGES\*\**Grand Oaks Nursing & Rehabilitation Center is seeking the top candidate and accepting applications for a highly motivated *Clinical Care Coordinator. *Choose a challenging and rewarding career with Peplinski Group's Nursing & Rehabilitation Centers, where you can enjoy a deep sense of personal satisfaction by making a positive and meaningful difference in the lives of others.*Wage Information:** $12,000 SIGN ON BONUS* $44.45/hour-$50.45/hour based on experience* Including MI pass through pay $0.85/hour until 09.30.25*Purpose of this position*The purpose of this position is to assess residents' physical and mental function and document data on minimum data set forms completely and accurately; document all additional assessments required completely and accurately; and determine appropriate referrals to other health care professionals; and to use the resident assessment protocols to determine whether to proceed or not proceed. This position will be responsible for the overall clinical oversight of the residents in his/her assigned unit. To Identify a key person on the infection prevention team having responsibility for the infections prevention program, dissemination of infection prevention information including surveillance data and policy decisions.*Job Overview** Interview residents, residents' representatives and others with information related to the minimum data set assessment.* Collect all data necessary to complete minimum data set forms and related documentation.* Make appropriate referrals to other health care professionals.* Participate and/or conduct interdisciplinary meetings.* Supervise and participate in the overall care of the residents* Oversee clinical coordination, review labs, assist the physician with rounds* Participate in the facilities Quality Assurance process to review facility systems as directed* Effectively direct, manager, and operationalize the infection prevention program*Requirements for this position** Registered Nurse RN* Great customer service* Strong interpersonal and time management skills* Successful candidates will complement existing team members.*Our Mission*Our valued employees are central to our mission of providing excellent care.
