Scheduled Hours40Position SummaryWe are looking for a dedicated nurse to provide comprehensive care and support to pregnant individuals with HIV and their newborns post-delivery. This is a position that covers the St. Louis Metropolitan Region and works with Adult HIV and OB providers and Pediatric Infectious Disease Specialists for the newborn within Washington University, SSM, SLUCare, as well as other providers. The nurse will collaborate with Ryan White Part C and D clinical HIV care teams to enhance engagement, retention, and health outcomes for pregnant individuals with HIV, from pregnancy through the postpartum period, as well as for their infants exposed to HIV. The role focuses on implementing and assessing innovative strategies to maintain ongoing engagement in HIV care, aiming to achieve viral load suppression and prevent perinatal HIV transmission.Job DescriptionPrimary Duties & Responsibilities: Coordinate with patient chosen Adult HIV and OB Providers of each enrolled patient to:
  • Assist in monitoring individualized treatment plans to support treatment adherence and viral load suppression.
  • Schedule ancillary medical care visits and provide follow-up to ensure appointments are kept.
  • Work with pharmacies to ensure prescribed medications are received.
  • Track health outcomes, laboratory results and procedures.
  • Provide treatment adherence education.
  • Perform other activities for effective medical management of patient needs.
Actively collaborate with multidisciplinary team (including case managers, peer educators, mental health specialist, psychiatrists, and primary practitioners) to:
  • Develop care engagement plans, and monitor and update plan goals.
  • Offer on-call clinical consultation to team members.
  • Use EPIC to document activities and communicate with providers regarding the care plan.
  • Organize monthly meetings with the multidisciplinary team to maintain patient care and ensure team collaboration.
  • Coordinate with SSM Cardinal Glennon Pediatric Infectious Disease providers to inform them of expected upcoming deliveries at SSM.
  • Ensure that patients' preferences for feeding, delivery, and overall care are communicated to all relevant teams.
  • Establish and maintain relationships with agencies outside of our Target Area to ensure patients receive equitable care regardless of their location.
Conduct medical services through home and community visits to:
  • Evaluate the impact of the home environment on the health of the mother, baby and family.
  • Offer health education and referrals based on household needs.
  • Collect lab work as necessary if there are barrier to doing so at the clinic site.
  • When needed, make visits to homes or medical appointments to make necessary contact with the patients (i.e., delivering medication from the pharmacy, taking out resources, possible lab work if needed).
Support clinical quality management and service evaluation efforts, including:
  • Engage in quality improvement activities in accordance with the site's quality management plan and designated performance indicators.
  • Ensure accurate and timely data entry in all relevant databases and electronic record systems.
  • Review medical records of enrolled patients to verify that care provided aligns with established HIV/AIDS clinical guidelines.
Provide HIV prevention and care education to patients, their partners, and family members:
  • Provide counseling regarding HIV prevention to patients and their sexual partners.
  • Refer partners to behavioral and/or bio-medical HIV prevention interventions including PrEP.
