**Overview** *****This position is remote within California, Nevada or Arizona (with the ability to possibly travel for meetings), with a clear and current CA RN license.** The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric full-service management service organization structure. As the Director of Care Coordination, the candidate will be integral to managing care coordination for 2.6 million attributed lives, providing leadership that directly contributes to our goal of supporting value-based care across our extensive provider networks. The Director will have the unique opportunity to lead and shape a team focused on care transitions, patient advocacy, and clinical social work, working in close partnership with a broad range of departments and external partners. This collaborative approach ensures seamless patient transitions through the healthcare continuum, enhancing the quality of care and patient experience. The role is embedded within CommonSpirit's Population Health Services Organization (PHSO), which centralizes expertise and resources to support value-based care. With a direct report to the Chief Physician Executive, this role is positioned within a forward-thinking structure designed to standardize high-quality tools and services across markets, enabling care coordination to thrive on a national scale. The Director will play a pivotal role in implementing best practices for discharge planning, clinical social work, and out-of-network reviews. This involves both strategic and operational responsibilities, offering the opportunity to innovate and improve patient care models in ways that are highly scalable, evidence-based, and reflective of cutting-edge healthcare trends. Managing regulatory and government reporting requirements, this role is essential to ensuring compliance with Federal and State standards, as well as aligning with CommonSpirit and DHMSO policies. The position requires someone detail-oriented and capable of driving high standards to maintain compliance and ensure the organization's reputation for excellence. **Position Summary:** Under the direction of the Chief Physician Executive, the Director leads the Care Coordination teams, including but not limited to clinical resource management, partnering with the inpatient departments for discharge planning activities, patient advocacy, clinical social work and best practice in out of network review. Responsiblities may include: - Directs and manages areas including planning, organizing and directs all activities, staffing, performance improvement in the delivery of clinical services (such as care planning). Reports needs within Dignity Health Medical Service Organization (DHMSO), as well as government and regulatory reporting. - Guides Care Coordination activities according to the needs, requirements, and policies of DHMSO, the affiliated medical groups and health plans, Common Spirit Health (CSH), any Federal and State agencies, and according to standard practices of the professions under the director's accountability. - Consults and collaborates with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature. - Works closely with all departments at DHMSO and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge. **Qualifications** **Minimum Qualifications:** - Minimum of three (3) years progressive supervisory or management experience in an acute hospital setting. - Current CA Registered Nurse (RN) license. - Bachelor's degree in nursing or experience equivalent required. **Preferred Qualifications:** - CM certification preferred - Previous care coordination experience strongly preferred. - Experience in a value-based setting preferred. \#LI-DH \#carecoordination \#ccleaders **Pay Range** $54.12 - $78.47 /hour We are an equal opportunity/affirmative action employer.
