Job Description :
We are an acute care medical center looking for a RN for our Director of Case Management available position. As the RN, Director of Case Management you will be responsible for the administration, direction and evaluation of Case Management services, as well as, developing strategies for the advancement of case management, to include utilization management and coordination of care and transition management. As the RN Director of Case Management you will need to demonstrate, through action and plans, that there is consistent standard of excellence to which all department work is expected to conform. Such standards are based on establishing and maintaining a consistency of purpose, focusing on continuous improvement within the Director's area of influence, and delivering the highest degree of quality service possible.Position: RN, Director of Case ManagementLocation: Brooklyn, NYSalary: $140,000 – $170.000Responsibilities:Directs case management and social work activities to monitor quality of care, verify appropriate documentation and compliance with standards.Addresses and resolves patient/family and staff satisfaction issues on an ongoing basis.Identifies and intervenes about specific LOS (length of stay) issues on an ongoing basis.Develops and evaluates case management for high risk/high volume patients.Implements and evaluates concurrent utilization review practices.Effectively manages concurrent denials.Utilizes data to improve department activities and performance.Collaborates with other department leaders to assure the smooth integration of Case Management/Social Work services across the continuum.Monitors and maintains JCAHO, federal and state regulatory readiness.Critical responsibilities include achievement of annual and periodic goals for significant indicators of performance.Requirements:Bachelor's Degree in NursingMaster's Degree in Nursing and other health care related courses – PreferredMinimum of five (5) to ten (10) years acute care clinical experience.Minimum of five (5) years case management leadership EDUCATION:New York State Licensed Registered Professional NurseCertification in Patient Review Instrument (PRI) and ScreenCertification in Case ManagementCertification in Hospital Management – HIGHLY PREFERRED
