RN Float Case Manager – Cardiac and Oncology Service Lines - WC - Days The Float Case Manager will provide comprehensive case management support for both the cardiac and oncology service lines. This role involves coordinating and facilitating discharge planning for patients transitioning home, ensuring continuity of care and optimal patient outcomes.Key Responsibilities
- Collaborate with interdisciplinary teams to develop and implement individualized discharge plans for patients.
- Coordinate home care services, including nursing, physical therapy (PT), occupational therapy (OT), and infusion services.
- Assess patient needs and ensure appropriate resources are in place to support a safe and efficient transition to home care.
- Communicate with patients, families, and caregivers to provide education and address concerns related to post-discharge care.
- Serve as a liaison between the hospital, home care agencies, and other community resources.
- Maintain accurate and timely documentation of discharge plans and follow-up care.
- Flexibly support both cardiac and oncology teams, adapting to the dynamic needs of each service line.
This position is ideal for a proactive professional passionate about providing high-quality transitional care across diverse patient populations.Preferred Criteria
- 2 years' of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
- 1 year of leadership experience in a healthcare setting
- Master's Degree in healthcare related field
Required Criteria
- Bachelors degree in Nursing (BSN)
- Current NYS Registered Nurse (RN) licensure
- 3 to 5 years of health care experience in an acute care setting
- Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
