Job Overview

TheHospice RNprovidesdelivery of quality hospice nursing care for patients in a variety of settings, including patients under home health and hospice plans of care,residingin private homes, assisted living, skilled nursing facilities and hospice inpatient facilities.Hospice RNcareforpatientand families that is reflective of the organization's core values. TheHospice RNis knowledgeable about and supportive of the primary nursing model.

Job Description

Minimum Qualifications:

1. Massachusetts RN Licensure. 

Hours: 32 Hours

Shifts Needed: 3-11PM or 11PM-7AM

Location: High Pointe House, Haverhill MA

Duties and Responsibilities:Theduties and responsibilities listedbeloware intended to describe the general nature of workandare not intended to be an all-inclusive list.Other dutiesand responsibilitiesmay be assigned.

• Provideshospice nursing care to patients and familiesutilizingcurrent principles of palliative care and symptom management to produce hospice outcomes.

• Utilizesnursing process and current standards of nursing practice inprovidinghospice care.

• Remainscompetentontechnical nursing skills (i.e.IVs,Gtubes, etc.)

• Understands family dynamics and works effectively withinvarious typesof family systems.

• Utilizesresources appropriately,demonstratesthe ability to act as a patient and family advocate andprovidesinformation to families about all available resources.

• Accurately addresses need foradditionalservices. Communicates patient reports and status changes concisely and effectively.

• Assessespatient for need for Fall Precautions. Updates Care Plan, Medication Profile and HHA Treatment Plan routinely and as patient condition changes.

• Completes all nursing documentation per procedures and nursing standard of practice including assessments, interventions, responses to interventions, communications, verbal orders, etc.

• Involves Team Leader and Medical Director in situations appropriately.

• Understands how tomaintainprofessional boundaries with patients and families.

• Assists in orientation ofnew staffandmentors LPNs and nursing students asrequested.

• Pursues standards of excellence in performanceas a Hospice RN.

• Works with Team Leader and Clinical Manager toidentifyopportunities for improvement in performance and works to address them.
