Available territories: Camden or Gloucester Counties What you will be doing: • Assessment: Home care RN case managers conduct comprehensive assessments of patients' health, needs, and living conditions to develop tailored care plans. • OASIS Documentation: They utilize the Outcome and Assessment Information Set (OASIS) to document patient data, ensuring accurate and standardized information for effective care coordination. • Clinical Care: In addition to OASIS documentation, home care RN case managers actively provide clinical care, including administering medications, wound care, monitoring vital signs, and offering direct medical support to patients in their home environment. • Coordination: Case managers coordinate and communicate with healthcare professionals, caregivers, and support services to ensure a holistic and seamless delivery of home healthcare services. • Monitoring and Evaluation: Continuously monitor and evaluate patients' progress, adjusting care plans as needed.
