A healthcare organization in Western New York is currently seeking a licensed Registered Nurse (RN) to join their team in Erie County as a Facility Based Hospice Nurse. In this role, the Hospice Admissions Nurse (RN) will be responsible for performing professional nursing services, in accordance with physicians' orders, for individualized patient care and psychosocial/spiritual interventions for assigned patients/families. About the opportunity:Schedule: Monday to Friday, (1) weekend every (4) weeks Hours: 8am-4pm Setting: Facility based (Hospice) Responsibilities: The Facility Based Hospice Nurse (RN) will:

• Conduct comprehensive assessments to address physical, psychosocial, and spiritual needs related to terminal illness, ensuring patient well-being and dignity
• Perform pain assessments and ensures optimal pain relief through interdisciplinary interventions
• Prioritize assignments to meet productivity and documentation standards within defined timeframes
• Educate interdisciplinary teams on disease progression and symptom etiologies
• Anticipate and addresses patient safety issues and symptom control challenges by modifying care plans promptly
• Demonstrate knowledge of medications, including usage, side effects, and interactions
• Adhere to agency standards for documentation and regulatory compliance
• Maintain patient and organizational compliance with policies, procedures, and HIPAA regulations

• Graduate of an accredited School of Nursing
• Current NYS Registered Nurse (RN) license
• Solid assessment, clinical, and documentation skills
• Working knowledge of Electronic Medical Records (EMRs)
• Microsoft Office Suite proficient

• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

• 1+ years of Nursing experience
• Bachelor's Degree in Nursing
• Certification in Hospice and Palliative Care Nursing
