Department Description: This position plays a crucial leadership role by coordinating outcomes-driven business growth. Position works with a dedicated team including an administrator, clinical management team and marketing and sales support.This is a clinical business development position that is responsible for generating, managing and coordinating referrals and transitions of care from key hospitals, skilled nursing facilities and other community settings in the area; cultivating and maintaining productive professional relationships with referral sources; maintaining industry knowledge of best practice and emerging trends; and providing education for health care professionals and other community partners on hospice and palliative care services.Qualifications* Current RN licensure or compact licensure recognized by the State of Vermont.* Minimum 5 years of working in hospice field.* Hospice & Palliative Care Certification preferred or willing to be certified within 1 year.* Advance training in Serious Illness Conversations preferred (TalkVermont as example)* Sales, marketing and liaison experience preferredOn-call: Not scheduled.
