15611BRTitle:RN Out PatientDepartment/Cost Center:808 - Strong Health Geriatric GroupJob Description:Interview the patient and/or caregiver in preparation for the in person geriatric assessment. Telephone or two-way video conferencing with patients and/or caregivers and the care team in follow-up care plan interventions.RN is primarily responsible for performing telephone preparation for visit and collaborative geriatric care plan development and implementation. Additionally, the RN will preform in home visits to assess medical and care plan interventions and complete documentation of visits.Additional Essential Duties:Communication with the primary care team regarding changes in patient status and care plan.Participation in interdisciplinary care team meetings for development and follow up of care plans.Attend and participate in practice quality assurance and practice improvementSalary Range:$34.01- $39.43The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.Auto req ID:15611BRJob Requirements:Fulfilled the educational requirements to be a licensed RN in NYS. 3-5 years experience. Office Practice and/or Geriatrics experience preferred. Current New York State licensure as a registered nurse; CPR Certification.
