OverviewWe are seeking a dedicated Registered Nurse (Case Manager) to join the Utilization Review team at WA PCH Centralia Hospital. This 13-week assignment offers the opportunity to contribute to patient care coordination in a dynamic hospital environment.

Description :

• Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
• Serve as the primary liaison between patients, families, and healthcare providers to ensure seamless communication and care delivery.
• Coordinate and manage patient transitions between inpatient, outpatient, and community care settings.

Utilization Review

• Assess and evaluate patient care needs to determine appropriate levels of care.
• Monitor resource utilization to ensure compliance with hospital policies and payer requirements.
• Perform pre-certifications, authorizations, and concurrent reviews to align with payer guidelines and avoid denials.
