$40 per hourPosition Scope:Under the direction of the Director of Nursing of the Skilled Nursing Facility, this position is responsible for the management and supervision of assigned nursing unit. Qualifications/Education/Experience: Graduate of a registered professional nursing school.BSN and Masters in Nursing Administration preferred Current NYS Registered Professional Nurse LicenseKnowledge of PRI, MDS, and Quality Indicators. Two (2) years of Geriatric Nursing or Long Term Care Supervisory experienceResponsibilities:Establishes daily priorities, tasks and communicates to the nursing staff. Assists nursing staff, residents, families and other team members for assistance in identification of resident needs.Conducts daily independent rounds with other appropriate team members to observe and assess resident care, needs and outcomes. Ascertains and secures the resources necessary to meet the resident's needs.Develops assignments for resident care with unit team input (licensed and unlicensed) based on resident and unit needs. Initiates, develops, supports and contributes to the Interdisciplinary Care Plan and attends the Care Planning meetings. Assures MDS is completed in an accurate and timely manner. Assures all personnel involved in providing care to the residents are aware of the care plan.Attends and participates in continuing educational programs designed to keep abreast of changes in the nursing profession. Maintains skills necessary to monitor, oversee and assist as needed with medications, therapeutic treatments, lab work, EKG's, x-rays, IV therapy, oxygen therapy and other clinical procedures as necessary.Upholds established standards for work performance and evaluates individual work performance through observation of care given, evaluation of resident outcome, professionalism displayed, and compliance with proper documentation of skills providing performance feedback. Monitors and assures emergency equipment is in a readiness state and that staff are trained to handle resident emergencies effectively and quickly.Communicates all pertinent information to alternate shifts, physicians and with appropriate departments. Reviews all medical records and documentation on a regular basis to assure accuracy and completeness. Adheres to established infection control standards in the performance of all duties as outlined by facility policy, State and Federal mandates.Reviews complaints, grievances, incidents and accidents, unusual occurrences, conducts an investigation and makes an oral and written report to DON and Administrator offering solutions to prevent similar occurrences.Assures that all unit staff maintains resident rights and dignity at all times and provides resident care in a consumer friendly manner. Assures residents call lights are answered promptly and appropriately.Monitors and ensures an appropriate, pleasant and timely meal delivery, environment and feeding assistance program. Monitors and prevents resident abuse, neglect and mistreatment.Relates to ancillary staff assigned to unit related to the day to day unit functioning to ensure resident needs are met promptly and satisfactorily. Assists to develop training protocols specifically related to short term rehab with physical therapy and other ancillary departments as needed. Ensures both current staff and any new staff have completed and understand all short term rehab protocols.Evaluates growth of unit programs through census occupancy, satisfaction of residents and achievement of resident individual goals. Interviews, hires unit staff as needed. Completes timely performance evaluations through review of job skills and observation of resident care and evaluation of resident outcomes. Ensures that all unit staff is following their respective job descriptions and facility policies and procedures. Disciplines staff as needed following disciplinary protocols and union contract. Uses HR and DON as resources.Assists in orientation of new nursing staff in conjunction with Staff Development. Sets up mentor and their schedule to orient new employee. Oversees completion of the orientation on the unit. Assures opportunities for and participation in continuing education programs for staff. Works together with Staff Development for compliance of all staff regarding state mandated required in-services. Makes appropriate reports/audits to the DON as required or as may be necessary. Assures that all unit employees follow the requirements of the LutheranCare Safety Program.Participates environmental rounds and develops and implements corrective actions.Monitors and investigates the CMS Quality Indicators triggered for their unit residents. Formulates creative plan of action with care plan team to improve resident outcomes and decrease quality indicators. Such as decreasing restraints, falls - preventing sentinel events. Submits and meets with DON to discuss plan of action.Monitors clinical/unit trends and areas for improvement through observation and reports/audits and works with nursing administration and other disciplines to create corrective policy action and resident care improvements. Will lead and/or participate in these Quality Improvement Teams.Performs other related duties or miscellaneous duties as assigned.
