Summary of Job Purpose
- Responsible for evaluating and recommending wound care treatment of the inpatient and/or the outpatient. Types of patients seen by this service include the critically ill patient, in accordance with accepted protocols.

- The RN must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served
- The RN delegates and collaborates with other staff members
- Demonstrates critical thinking and independent decision making; utilizing policies and procedures, standards of care and data analysis
- Collaborates and communicates with physicians to ensure quality care
- Develops utilizes and models effective time management skills.
- Plan for appropriate patient populations and manages plan of care with interdisciplinary team through ongoing collaboration and use of care plans. Participates in development of plan.
- Communicates with all team members regarding therapeutic interventions including pt./family/significant other
- Acts as clinical resource and mentor for staff
- Delegates appropriate tasks to licensed staff, monitoring and maintaining accountability for the quality of care
- Manages resources, which promote cost effectiveness, efficient operation of unit and positive outcomes
- Initiates and communicates change in patient care delivery based on continual evaluation of progress according to outcome-based care plan
- Provides input into employee feedback process for the other members of the staff including the Outpatient Wound Center and the Medical Director
- Initiates collaboration with physician and interdisciplinary team of pertinent pt. Information to ensure continuity of care
- Participates in education and training for the staff.
- Coaches and provides feedback on an ongoing basis to all team members
- Makes recommendations to improve patient care delivery process
- Performs any other duties as assigned
- Out-Patient
- Participates in tracking wound cultures and timeliness of reporting results to physicians
- Performs comprehensive patient assessments, anticipating potential care problems and prioritizing care needs in order to effectively provide optimum care
- Sets patient care priorities
- Coordinates and communicates outpatient plan of care with patient(s), family and outside agencies and interdisciplinary team
- Participates in maintaining inventory control
- In-Patient
- Gathers monthly data on nosocomial pressure ulcers.
- Leads Skin Saver Committee occasionally at the request of the Wound Care Manager.
- Enters focus study data on skin integrity concerns in Midas Program.
- Responsible for all skin/wound referring consults and making appropriate recommendations, including wound vacs and starting wound vacs and doing wound vac dressing changes.
- Gathers monthly data for bed rentals and patients discharged with coded pressure ulcers.
Essential Job Qualifications
- Education:
- NYS RN License required. Bachelors Degree in Nursing preferred.
- Experience:
- Critical Care Nursing Exp. (preferred); Wound Care Exp (Preferred)
- Training/License:
- Registered Licensed RN required. BLS Certification; ACLS certification strongly recommended
