Under the direct supervision of the Wound Care Nursing Administrator and Program Director, provides patientwith care that is continuously evaluated, planned for, and revised as needed; patient's status isdocumented within the reassessments; patient care needs; interventions to meet the patient's needs;nursing care provided; the patient's response to; and the outcome of care provided; organizes andprioritizes work to provide health information, discharge planning, and patient care in a safe environmentto maximize health in the continuum of care and protect the patient; assumes responsibility for andassigned group of patients and be accountable for an assessment of the patient's basic human needs,enabling team of providers to make appropriate decisions to develop, implement and evaluate a planfor care, to include biophysical needs, bio-psychological needs, environmental/safety needs, self careneeds, educational needs, discharge planning needs; professional and competent as a care provider, aleader of personnel, and a collaborator with other professionals; demonstrates awareness of his/herunique position in the Department and responsibilities as a professional nurse and R.N. Case Managerthrough accurate, clear, concise and timely written and oral communications.

Job RequirementsMinimum EducationAssociate of Science in Nursing- RequiredB.S.N. PreferredRequired Licenses[Kentucky, United States] Registered NurseKentucky State RN LicenseMinimum Work ExperienceAt Least 1 Year Wound Care ExperienceBLS and ACLS Required within 3 months of hire. PALS Preferred but not required.CWON, WOCN, CWS, WCC, CHRT, or CHT preferred.
