Community Care, Inc., a leading provider of home health care services is seeking a detailed-oriented and highly organized Scheduling Coordinator to join our team. The Scheduling Coordinator pays a crucial role in ensuring the seamless delivery of care services to our patients by efficiently coordinating schedules for our dedicated team of home health professionals.

Key Resposibilities:

  • Create and manage schedules for home health aides, nurses and other team members to ensure all patient visits are properly staffed and scheduled.
  • Match the skills and qualifications of our team members with the specific needs and preferences of our patients.
  • Communicate with patients and their families to coordinate visit times and ensure their satisfaction with our services.
  • Handle last minute schedule changes, call-offs and emergencies with a focus on maintaining continuity of care.
  • Collaborate with clinical staff to ensure that care plans and schedules align with patients' healthcare needs.
  • Maintain accurate and up-to-date scheduling records and patient information in our system.
  • Assist with recruitment and onboarding of new team members.
  • Maintain compliance with all applicable laws, regulations and company policies.

Qualifications:

  • High school diploma or equivalent (Bachelor's degree preferred)
  • Previous experience in healthcare scheduling or a related field is highly desirable.
  • Strong organizational skills with excellent attention to detail.
  • Exceptional communication and interpersonal skills.
  • Proficiency in computer software, including electronic medical records.
  • Ability to work under pressure and handle scheduling conflicts with diplomacy and efficiency.
  • Knowledge of home health care regulations and processes is a plus.
  • Compassionate and customer-focused mindset.
  • Ability to work independently and as part of a team.
