Job Summary:
We are seeking a dedicated and detail-oriented Scheduling Coordinator to join our healthcare team. The ideal candidate will manage new referrals, ensure timely case openings, match workers to cases, and oversee patient care continuity. This role also involves addressing employee performance issues and ensuring departmental standards are met.
Key Responsibilities:
- Process incoming referrals promptly and efficiently.
- Match the best workers to cases, prioritizing patient needs and care continuity.
- Collaborate with contract case managers and external contacts to enhance the agency's reputation.
- Notify admitting hospitals and facilities of the agency's role as the patient's home care provider.
- Encourage departmental growth through networking and relationship-building.
- Address patient complaints and cooperate with management to improve workflows.
- Maintain accurate and timely documentation, adhering to HIPAA regulations.
- Monitor employee performance and report issues such as no-shows, schedule deviations, or unreliability.
- Print and distribute accurate weekly schedules for paraprofessionals.
- Review and file all paperwork, ensuring completeness and compliance.
- Participate in orientation and in-service programs.
- Support quality assurance processes and contribute to the development of improved scheduling systems.
- Ensure the agency's disciplinary policies are enforced fairly and consistently.
Qualifications:
- High school diploma or equivalent.
- Minimum of one (1) year of recent experience as a scheduling coordinator in a home care agency or related healthcare field.
- Strong multitasking skills and a passion for working with and supporting people.
- Ability to pass a criminal background check.

- Proficiency in maintaining secure and compliant patient records.
