Job Description :
Position SummaryThe School Based Facilitator provides skills training and case management services to the identified client. They provide therapeutic services identified in the service plan by the client and family to assist the client in their recovery. The School Based Facilitator collaborates closely with school personnel and acts as an active treatment team member. They provide support and collaboration to school personnel on support and resources available.Essential Duties and ResponsibilitiesThe essential functions include, but are not limited to the following:
  • Lives out the INcompass Healthcare promise by consistently displaying compassion, treating all with dignity, and providing exceptional care for everyone.
  • Incorporates the six principles of trauma informed care throughout all aspects of their work.
  • Provides direct treatment, such as teaching skills, case management, coordination of care, empowerment, and crisis intervention in accordance with agency policy and procedure.
  • Develops and teaches skills that would include social skills, emotion regulation skills, coping skills, mindfulness, communication, and relationship skills, etc.
  • Becomes familiar and maintains working knowledge of funding source requirements.
  • Finds value in collaboration with all treatment team providers and fosters team collaboration to ensure the client is successful in their treatment.
  • Develops and fosters effective communication between all school personnel and the family.
  • Acts as a client advocate and empowers them in their treatment.
  • Coordinates communication and services provided by internal and external programs and agencies.
  • Completes administrative tasks, such as attending training, meetings, serving on committees as assigned, participating in school and community activities, engaging in supervision as identified and according to policy.
  • Complies with documentation requirements according to agency policies and procedures.
Minimum Qualifications (Knowledge, Skills, and Abilities)
  • An associate degree in human services, social work, psychology, sociology, or related field and/or equivalent experience (professional and lived experience).
  • Preferred bachelor's degree in human services, social work, psychology, sociology, or related field.
  • Demonstrates effective verbal and written communication skills in a professional setting with adults, caregivers, and professionals, such as teachers and administrators.
  • Ability to demonstrate effective engagement skills.
  • Ability to work independently.
  • Exceptional organizational and time management skills.
  • Ability to build and maintain relationships with community partners.
  • Demonstrates knowledge and skills related to specific populations as applicable.
  • Ability to work as a treatment team member.
  • Knowledge of IEP's and behavioral plans.
  • Demonstrates skill and sensitivity to cultural differences.
  • Demonstrates effective decision-making skills, problem solving, critical thinking skills.
  • Ability and willingness to travel on behalf of agency behavior and work in a client's home and community setting.
