They provide ongoing social services to program participants to secure long-term housing stability and prevent shelter entry. In addition, they assist Program Director with program management and staff supervision when needed. Specific duties and responsibilities include: Coordinate services and activities to provide program participants with necessary tools to sustain their housing and prevent homelessness. Complete a full case assessment and determines clients eligibility for the Homebase program. Develop and implement an individualized service and housing plan that establishes goals and actions which address the immediate housing crisis and the underlying causes of the crises. Monitor and evaluate program participants progress on the implementation of their service plan. Prepares all necessary documentation to prepare in-house subsidy applications on behalf of the client. Provide referrals for clients to access public benefits and entitlements. Liaise with the Assistant Director to determine if legal referral is necessary to prevent homelessness. Maintain accurate record keeping via case files and the designated computer database. Engage in the arrangement, coordination, monitoring and delivery of services to Homebase program participants. Explain the RiseBoro Homebase process to new clients and provide compassion and reassurance. Assess households for long-term City, State and Federal rental assistance program.Relieve the clients of trepidation and serve as the voice of knowledge in each case regardless of complexity. Develop linkages with community-based organizations. Train and support Case Managers in database utilization, chart organization, and writing/collecting documents for FARS. Review Case Managers FARS and submit to Assistant Director for approval.Keeps abreast of all resources available for Homebase clients including HRA policy and procedures, etc. to ensure excellent services provision. Conduct chart reviews and designated case review assignments, where necessary. Provide day to day guidance, support, trouble-shooting, leadership, and improvement strategies. Participate in management meetings to address program challenges. Participate in the ongoing development of the program and center.
