A Senior Patient Navigator, ACO, assists the Administrative Director in providing leadership to maximize the health status of patients and improve the clinical experience and satisfaction of services for patients, physicians, and staff. This position requires the ability to independently function as a resource for the management and staff in order to expand the quality and efficiency of ACO services and programs. Advanced nursing knowledge and population health expertise are essential to identify and implement both clinical and IT improvement processes. The ability to influence, direct, and then monitor work flow processes to identify and overcome barriers is central to success in this position.

This hybrid position, 2 days a week, but it can change based on the department needs.

A day in the life of a Senior Patient Navigator, ACO at Hackensack Meridian Health includes:

  • Demonstrates effective leadership skills by exhibiting in depth knowledge of ACO systems, functions, and implementation of consistent accountability methods.
  • Direct responsibility for development, implementation, and monitoring of new programs. Co-manages programs with the Administrative Director and provides solutions to barriers to ensure overall program success.
  • Directly involved with the development and enhancement processes of the ACO with the aim to improve the clinical experience and satisfaction of services for referred patients and/or physicians.
  • Expert in all aspects of ACO IT software. Conducts on-going meetings with software vendors to improve system capabilities and efficiencies geared towards improved patient care and user satisfaction.
  • Responsible for training and acts as a continuous resource for ACO staff and physicians on essential ACO software such as Healthy Planet and Patient Ping.
  • Trains and mentors new ACO staff with the creation and continued enhancement of a standardized training program.
  • Trains and mentors new ACO practices with a standardized orientation program and continued assistance with work-flow development.
  • Functions as a resource to the ACO staff and practices. Identifies educational/training needs and develops solutions with the Administrative Director.
  • Participates in vendor management by communicating ACO staff and practice needs then follows through to ensure needs are satisfied.
  • Act as a liaison between the care coordination staff and management to enhance communication.
  • Assists the Administrative Director and other management in the creation and updating of standards/policies, protocols, performance appraisal, and general problem identification.
  • Ability to effectively deliver live presentations to large and small groups both internally and externally.
  • Provides on-going updates on programs and policies to staff and practices. Collects and reports data as deemed necessary and by request of management to analyze possible gaps in care, focus on quality improvement processes and to track status of patient care.

Education, Knowledge, Skills and Abilities Required:

  • BSN.
  • Must have 8+ recent years in nursing.
  • 4 years recent ACO care coordination experience.
  • Highly developed interpersonal skills including motivational interviewing skills, in order to interact effectively and motivate staff and patients to change behavior when necessary.
  • Must be proficient at multi-tasking and prioritization working in a high volume environment with little supervision.
  • Ability to work independently as well as in a team environment.
  • Excellent analytical and deduction skills.
  • Good reasoning and problem solving ability and be able to take initiative in finding solutions to difficult and/or sensitive problems.
  • Must be organized and able to multi-task.
  • Ability to work in stressful situations and be flexible to accommodate patient and family needs.
  • Demonstrated proficiency and comfort with PC and Microsoft office skills.

Education, Knowledge, Skills and Abilities Preferred:

  • Experience in a variety of patient settings is recommended.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.
