We expect all staff to put our clients' needs first while respecting ourselves and each other as we provide hope to those who participate in our programs.TSINY, a non-profit mental health services agency, is seeking a Senior Placement Specialist for its Intake/Discharge/Finance Unit, in Jamaica (Queens), New York.Position SummaryResponsible for conducting intake screenings and coordinating admissions for TSINY's various programs. Play a key role in assisting individuals in need find appropriate services through TSINY. Responsible for ensuring that TSINY's programs are reaching the maximum number of individuals who can be served effectively. Play a role in working with the IADF Director to ensure that IADF duties are carried out in accordance with TSINY's policies, procedures, and mission.Essential Functions
Coordinate referral process (obtain documentation, follow-up with referring agent). Upon receipt of a referral, an initial intake appointment should be scheduled within one week. Enter screening assessments into AWARDS. Document daily progress and weekly contacts in AWARDS. Coordinate and process financial assessment forms. Provide Financial Specialist with any additional financial information/documentation required in a timely fashion. Obtain HRA 2010e status. Maintain appropriate and adequate charts on those individuals being referred to TSINY. Coordinate and prepare documentation for new residents admitted. Contact appropriate Program Director for the purpose of scheduling orientation meetings, i.e., tours. Coordinate, consolidate, and finalize residential census/statistical reports. Schedule and conduct outreach. Assist referral agents and other professionals with the preparation of referral data when necessary. Handle correspondence and telephone inquiries related to intake and/or discharge. Utilize and disseminate information from the unit's resource file. Assist residential staff with discharge planning. Be able to sit or stand as needed, with or without reasonable accommodation. May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation. Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation. During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
