The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay. We are seeking a Senior Program Manager to be responsible for providing administrative oversight of the Wellness Center Program operations to assure integration of preventative and support services with primary care services, including supervision of staff. Grant-funded: Contingent upon continuation of grant funding from year to year.
Duties and Responsibilities include but not limited to:- Responsible for the administration and supervision of the HIV Program/Wellness Center. Maintains positive working relationships with NYSDOH AIDS Institute and the federal Health Resources and Services Administration (HRSA) coordinating agencies, and program staff to assure positive program outcomes and contractual and work plan compliance.
- Assists and collaborates with the Wellness Director in overseeing, managing, and optimizing the operations of HIV, PrEP/PEP, and CoC programs.
- Develops and submits administrative reports, including: eHIVQUAL and monthly narrative report, and responds to inquiries from NYSDOH AIDS Institute BSFHC leadership team. Develops, promotes and maintains bi-directional referral arrangements with community health and human service providers.
- Attends all required meetings including Quality Assurance, and participates in Case Conferences (by phone or in person). Assures complete, timely and accurate recording of patient and programmatic data in electronic health record and NYSDOH AIDS Institute Reporting System (AIRS). Ensures implementation and administration of quality assurance and data collection systems to help evaluate program effectiveness, and makes recommendations for change.
- Meets monthly with Fiscal Department for review of budget and expenditures, assists Grants Department with development of new and continuing funding applications as necessary.
- Meets biweekly with Medical Case Managers, Data Manager, and clinical staff.
- Coordinates and maintains contact with other community organizations to establish community liaisons and linkage agreements. Attends planning and coordinating meetings with other agencies as required.
- Fosters support in the implementation of the HIV testing integrated services and partner contact programs.
- Assures that program staff attends training to support and update skills on health trends and changes.
- Reviews HIV educational curricula and materials and ensures that information is approved by agency leadership and NYSDOH AIDS Institute.
- Directs and supervise the staff, promptly addressing all program challenges in a proactive and solution-oriented manner. Provides input regarding staff performance for annual agency employment evaluations.
SKILLS:The successful candidate should have extensive experience working with individuals with HIV/AIDS, behavioral health, harm reduction and systems of care. Demonstrated experience in contract management.Excellent and effective communication, supervision, organization, and program management skills. Demonstrated ability to work effectively as part of an interdisciplinary team. Experience in harm reduction, clinical practice and health education. Knowledge of program development, management and evaluation. Computer skills, especially database and Microsoft Suite experience needed. Working knowledge of electronic health record keeping systems strongly preferred. Must be able to manage numerous projects and prioritize goals and objectives, while supporting the organizations mission and values.
EDUCATION:Bachelor's degree required Master's degree in health-related field such as MSN (Nursing), MSW (Social Work), MS (Psychology, Counseling), Public Health (MPH), or Public Administration (MPA) strongly preferred.
EXPERIENCE:Three-five years of progressive experience in program development and implementation. Extensive supervisory/management experience in a primary care clinic, hospital or community based agency. Experience in program implementation and budget monitoring. Demonstrated ability to work effectively in a team environment and exert positive leadership at all times.
