SENIOR SUPPORT SPECIALIST

Salary:$35,884.00 per year

SECTION I-JOB DESCRIPTION

SUMMARY:

Under the supervision of the Healthy Brain and Aging Program Director or designee, the Senior Support Specialist (SSS) will provide case management services and on-going support to elderly clients and their families through comprehensive assessments, planning, implementation and overall evaluation of family needs. The SSS will promote continuity of care for elderly clients and ensure their families are connected to the appropriate community resources. The SSS will serve as client advocate and program liaison in developing partnerships with other agencies to benefit the clients in the Healthy Brain and Aging Program.

ESSENTIAL FUNCTIONS:

* Maintains accurate and current documentation of case history, evaluations, assessments, needs, services, progress, financial data, and all other categories of information required in each client's

* Serves as client advocate and supports families and

* Responsible for direct intervention of case management functions for elderly clients in a timely

* Schedules monitoring services with the elderly client and family to discuss progress, concerns, plans and ensures the family makes informed

* Resolves case-related issues to achieve the objectives of the clients within the Healthy Brain and Aging

* Identifies family centered, community based, and culturally competent resources and makes them available to clients through

* Assists clients in the interpretation and completion of applications for social services assistance.

* Educates the clients and families about available resources and community

* Collaborates and participates with community coalition groups to identify and establish partnerships to assist clients and families to access

* Collaborates with staff to ensure adherence to program protocols and standards, federal and state policies, regulations and guidelines.

* Works with local, state and regional agencies in matters concerning the coordination of plans and

* Responsible for contacting clients as needed through various methods including telephone calls, mail notices and home

* Prepares and monitors program reports, analyzes data and identifies solutions to resolve barriers in clients accessing

* Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications and establishing personal

* Participates in coalition groups, community outreach events, health fairs, trainings, and educational workshops to establish working partnerships.

SECTION II-JOB REQUIREMENTS EDUCATION AND EXPERIENCE:

Licensed Baccalaureate Social Worker (LBSW) or Bachelor's degree in Social Work, Psychology/Sociology or an Associates in Social Work and two years of experience in case management services within community programs serving the elderly - OR - other qualified individuals with extensive and successful experience working in community programs serving clients with appropriate qualifications.

KNOWLEDGE, SKILLS, AND ABILITIES:

Advanced skills in making assessments of persons and situations with accuracy and clarity, to include appraising living conditions, such as physical and emotional environment. Knowledge of social work principles, ethics, methods, techniques, and practice of social work and/or counseling. Knowledge of appropriate health and social services in the community. Knowledge of human relations and group dynamics. Ability to accept responsibility and the utilization of sound judgment in making decisions. Ability to work cooperatively and effectively with other professional groups. Ability to prepare statistical/administrative reports and makes recommendations for program changes to work cooperatively and effectively. Ability to understand and subscribe to the philosophy and values of culturally relevant education empowerment and community integration. Ability to maintain confidentiality of clients enrolled in the Elder Pathways program and in the information and documents handled through program. Ability to work with minimal supervision. Ability to coordinate all aspects of administration through the Healthy Aging Director or designee. Ability to communicate effectively, using both oral and written methods of communications. Ability to plan and prioritize assignments. Ability to communicate effectively in Spanish/English to include reading/writing/oral. Skills in using Windows'2000, Excel and Power Point.

SPECIAL REQUIREMENTS:

Must have a valid Texas Driver's License, comply with State of Texas Driving Laws, and maintain an acceptable driving record. Must have reliable transportation. Employees of Cameron County may be required to work during any natural disaster or emergency, extended hours and non-traditional work schedules (i.e., Saturdays and Sundays). Criminal background check is required for employment or continued employment.

PHYSICAL DEMANDS:

Good health and physical condition sufficient to permit full performance of the duties of the position. If additional assistance is required, employee must request assistance prior to performing the assignment.

SECTION III-JOB DIMENSIONS

CONTACTS:

EXTERNAL CONTACTS:

Occasional contact with other health, social and educational care agencies and outside organizations. Frequent contact with CCPH staff and other county departments. Communication is via telephone, written communication and face to face.

INTERNAL CONTACTS:

Utilizes tact when dealing with County employees, health, social and educational care providers, and clientele regarding operations issues. Constant contact with Human Resources Office.

Communicating is primarily face-to-face, via telephone and through written communication.

RESPONSIBILITY:

Comply with Cameron County and Department of State Health Services rules/regulations and display a positive public image. As a Cameron County employee, the identity of every client shall be kept confidential except to the extent necessary to carry out the purpose of an investigation, hearing, or judicial proceeding.

DIFFICULTY:

Judgment is required to ensure completion of job activities as assigned by the immediate supervisor or designee. Uses discretion to achieve work goals. Initiative is constantly required to achieve work goals.

GUIDANCE:

As assigned by immediate supervisor or designee. It is the responsibility of the employee to know and maintain contact with their immediate supervisor or designee.

WORKING CONDITION:

Working conditions are primarily in a clinic, office, community center or field setting.
