Job Description:SCADD is seeking a compassionate full-time Service Coordinator at our Bank Street Transitional House location to promote engagement in treatment and facilitate an effective discharge plan whereby the individual successfully connects to the next level of care. Essential Duties and Responsibilities include but are not limited to the following.
  • Completes insurance verification and identifies payment resources, as well as assesses client eligibility for benefits.
  • Depending upon the program, facilitates the medication self-administration process and ensures compliance with the protocol required by the assigned department.
  • For programs without nursing departments, ensures that client medications are available and disposed of at the time of discharge according to the protocol required by the assigned department.
  • Manages any medical, behavioral, emergency, or outside problems quickly and efficiently by helping keep the environment safe and notifying the proper authority as needed.
  • Help answer the telephone, sort and distribute mail.
  • Prepare weekly and monthly reports, as necessary.
  • All employees are expected to participate in the orientation of new employees and the transfer of knowledge necessary for the performance of duties.
Education and/or Experience:Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Language Skills: Ability to read and comprehend complex instructions, correspondence, and memos. Ability to write effective and compelling correspondence to support position and/or recommendations. Ability to effectively present information in one-on-one and group situations to employees, vendors, partners, and stakeholders of the organization. Bilingualism a Plus!Full-Time Positions Offer:
