Duties:Provide home-based and remote case management to families with children ages 0-3 with developmental delays. Collaborate with the family to develop the Individualized Family Service Plan and outcomes for family supports, strategies and relevant services to be provided by our multi-disciplinary team of therapists and educators. Conduct initial and ongoing assessment of the child's development and functional skills, review progress, and outcomes to determine the progress made by each client while receiving early intervention services. Identify and refer families to community resources that support IFSP outcomes, address immediate family needs, or support natural learning opportunities. Ensure that all events and documentation are completed within the program's required timelines.Requirements:Bachelor's Degree in Early Childhood Education, Child Development; Early Childhood Special Education, Social Work, Psychology, Communication Disorders, Nutrition or a related EI field.Bachelor's Degree in a non-related field and 3+ years' experience working with or caring for infants and toddlers with special health care needs and their families. Or a High School diploma and 6+ years' relevant experience with approval from EOHHS.Meeting Street believes the dignity of each person is paramount.
