SHOPP's mission is based on the philosophy of self-help. We believe that older adults are capable of helping themselves and helping others improve their capacity to deal with the physical, emotional, social, and financial challenges they face as they age. SHOPP has several service programs in the Bronx that includes Senior Network Group Work Program; Community Care Network Program; VIP Elder Abuse Program and Crime Victims; NY Connects of the Bronx; Casa Boricua, Leon, Guess and Soundview Older Adult Centers; Case Management & Friendly Visiting Program; Caregivers Program and Lafayette Estates NORC Program. SHOPP's staff must be prepared to utilize the agency's philosophy of self-help and mutual assistance to enable participants to gain and express positive, psychological, and social well-being as an outcome of their involvement in SHOPP programs. SHOPP participants are provided with opportunities to: access benefits and entitlements; serve as volunteers; join planning and support groups; take advantage of training programs, participate in nutritional, educational, recreational and health promotion activities; and participate in community service and leadership development. The Service Coordinator is responsible for the provision of access and senior social, recreational, cultural, and recreational services to SHOPP Community Care Network at selected group sites and virtually. The Service Coordinator reports directly to the Program Director and Assistant Director. The position requires an associates degree in human services or related degree and experience in a social service setting. High School diploma with 3+ years of experience in social service setting and knowledge of older adult community-based services may be considered. The Service Coordinator is specifically responsible for the following tasks:
- Visit SHOPP sites on a weekly-scheduled rotation basis to conduct outreach and registration of clients and to monitor participation in programming;
- Screen clients for benefits, entitlements, and services;
- Coordinate with Social Service Teams to assist identifying participants in applying for needed benefits, entitlements, and services;
- Provide supportive counseling as needed in person and through Telephone Reassurance Calls;
- Provide information and referral to other appropriate agencies as needed and identify community resources for consultation and referral;
- Partner with the peer leaders, participants and program facility host staff to plan and implement a specially tailored program of classes, discussion groups and health promotion activities aimed at enhancing mutual aid and the strengthening of informal social support networks;
- Plan, develop and implement online virtual programming.
- Maintain participant case records, work logs and performance target outcome tracking, and statistical reports utilizing the funding source database;
- Participate in SHOPP staff training programs, staff meetings, Advisory Committee Meetings and other meetings as assigned;
- Recruit volunteers and monitor work logs;
- Develop partnerships and linkages with community groups;
- Complete other duties appropriate to the position as assigned by the Program Director/Assistant Director.
