Through a supportive relationship with the individual served, thorough knowledge of quality of life issues and community resources available, and the ability to access resources, the Service Coordinator ensures that individuals served receive assistance in gaining access to the services and resources that are requested.The job requires frequent use of initiative and independent judgment; aggressive intervention, frequent in person, long-term contact with individuals, families and service providers.Job responsibilities require travel within the service area for community-based service provision, and flexible hours and schedule.This position is expected to provide after-hours, holiday, and weekend emergency services as requested or required by the individuals receiving services.Essential duties include completion of comprehensive assessments of individual needs, desires, service planning, referral and linkage, monitoring, and advocating on behalf of individuals served; travel as needed; emergency after-hours response; documentation of services provided and interaction with the individual/family served and providers.Marginal duties include quality assurance improvement systems and community education activities.Job responsibilities are conduct in accordance with DADS Principles, Evidentiary Principles, Medicaid requirements. This position will report to the manager of Continuity of Services, Crisis Intervention and Enhanced Community Coordination. Requirements: Bachelor's degree or advanced degree from an accredited college or university in a social, behavioral, human services or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education or criminal justice. One year paid or unpaid experience working with individuals with IDD is preferred. Bilingual in Spanish and English a plus.
