A health services organization in California is currently seeking a new Service Coordinator to join their team in the Napa Valley. In this role, the Service Coordinator will be responsible for managing the delivery of services to regional center clients including those with intensive physical or emotional needs. About the Opportunity: State Date: ASAP Assignment Length: Contract to perm Schedule: Monday to Friday Hours: 7/9:30am to 5:30pm Patient Population: All ages Responsibilities: The Service Coordinator will: Write concise, accurate and objective Interdisciplinary Notes regarding the current status of the client and related service coordination activities on a monthly basis Communicate effectively in person, on the telephone and in writingRead, interpret, and follow complex policies, procedures, laws, and regulations Maintain current workload with accurate and complete documentation Conduct meetings, interviews and evaluations; interpret and document data Visit clients in their homes, schools, and day programs Perform other duties, as assigned Qualifications: 3+ years of experience working with the Developmentally Disabled population Bachelor's Degree in Social Work, Psychology, Counseling, Special Education, and/or Nursing Specific experience and/or education in one or more of the following areas: Social Work; Rehabilitation; Special Education; Child Development; Clinical Counseling; Genetics; or, Developmental Psychology Knowledge of human growth and development Knowledge of eligibility requirements for generic service organizations Knowledge of developmental disabilities and case management functions Solid assessment and documentation skills Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Bilingual Experience working with the IDD population
