Job Type Full-time Description OVERVIEW The mission of SGA Youth & Family Services is to help children, families, and communities facing great challenges realize their potential. The Trauma-Informed Behavioral Health Services (TIBH) program aims to address youth (ages 6-24) who have been exposed to violence in the Calumet City Cluster area with evidence-based treatment and services including therapy, service navigation, and community-based training on the signs and symptoms of trauma and mental health concerns. The Service Navigator will be responsible for helping identify and recruit potential clients for the program. They will provide connections to needed resources by the youth and their families to provide healing and stability. This could be referrals to housing, food, academic, job placement, medical, and/or transportation. The Service Navigator will follow the youth and family while enrolled in services, provide home visits, and ensure the clients receive identified resources. MAJOR DUTIES & RESPONSIBILITIES

  • Strategize effective ways to build relationships within communities to recruit, enroll, and help sustain caseloads.
  • Establishes and maintains MOUs with community partners to create a referral/linkage system for clients enrolled in the program.
  • Collaborates with the team to ensure comprehensive wraparound services for all enrolled participants to support participants in their recovery process.
  • Develops and maintains meaningful productive relationships with families, youths, and program community partners.
  • Meets participants in spaces that are most comfortable for them such as through home visits, school, or community locations.
  • Completes trauma screenings and needs assessments for 100 individuals annually to identify areas of need for the youth and family.
  • Provide referrals and resources for participants and support to ensure that they access the resources.
  • Document services are provided using ETO and other documentation as required by the program.
  • Develops and implements presentations/trainings/workshops based on the needs of the program and to ensure wellness.
  • Attends community network meetings and resource fairs/events targeting youth and families.
  • Completes other duties as assigned.
Requirements Education:
  • Bachelor's degree in social work, psychology, public health, human services, or other related field.
Experience:
  • Working with people in high-stress situations
  • Knowledge of resources within the Calumet City Cluster area
  • Experience with case management and working within a human service setting
Other:
  • Sensitivity to cultural diversity is required.
  • Ability to communicate and cooperate with diverse families, various professionals, and community groups.
  • Must be able to make home visits to present, prospective, and former participants in the participants' own homes.
  • The schedule may be flexible and may include some evenings and weekends.
  • Valid driver's license, access to a car, and willingness to drive.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees or the organization.
  • Exercises discretion and independent judgment in executing tasks.
  • Must demonstrate the ability to establish helping relationships with families of varying educational, experiential, and socio-economic backgrounds and handle emergencies and crises.
