FULL TIME POSITION:

FUNCTION & SCOPE:
To prevent child abuse and neglect and/or out of home placement and promote positive relationships
for youth ages 0-18 and their families.
ACTIVITIES, TASKS AND ASSIGNMENTS:
- Establish and maintain professional relationships with clients, service providers, funding sources and other staff
- As part of a team provide case management, crisis intervention and make and coordinate referrals to ancillary services
- Ensure all family members participate in a community program by connecting them with programs and maintaining on-going communication between family and program staff for participation success
- Communicate with and engage non-custodial parents offering them support services to assist them in reconnecting with the family/children
- Maintain regular communication with all family service providers to ensure seamless continuity of care
- Conduct weekly home visits and more as needed
- Schedule and participate in treatment reviews and other service provider meetings
- Facilitate psycho-educational groups as opportunity arises
- As part of a team complete service plans and treatment goals with clients
- As needed supervise visits with children and caregivers, and provide verbal or written reports to the Prevention Director and court system or funding source
- Participate in staff development opportunities, in-service trainings and contribute to the program's continuous improvement
- Ensure timely completion and submission of all required paperwork
- Flexible work hours to accommodate families and to attend community events that relate to the families the team is working with
- Build relationships with the schools to be able to monitor school attendance and assist in addressing any school issues.
- Communicate regularly by phone and in person with the C.A.R.E. program to ensure child/children's success
- Additional functions as determined by the Prevention Director

REQUIRED KNOWLEDGE, ABILITIES AND SKILLS:
- AA, BA preferred and minimum of one year direct experience working with children and families
- Experience working with individuals and families from different cultural and socioeconomic backgrounds
- Ability to demonstrate an understanding and appreciation of clinical concepts focusing on the strengths of the families you work with
Valid NYS Driver's License required.
