The successful candidate will:Provide psychological assessment, support and initiate patient referrals for proper management.Identify the psychological needs of the patients and to ensure the patients' needs and services are addressed.Collaborate with the Attending Physicians in the clinics for appropriate patient follow-ups.The overseer of the MICI Program staff and the logistical details.Functions collaboratively with the MICI staff on a daily basis for patient related updates.The proper management of the Providers' referrals and the completion of the required MICI forms.Provide the appropriate follow-up of the high risk patients for compliance measures.Provide the patients' documentation in the HealthBridge system.Required Qualifications: New York State Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW).Excellent communicator with a willingness to collaborate with interdepartmental, institutional and community partners. Preferred Qualifications: 1 to 3-years of post-masters experience in a hospital or health care setting.Acquired experience with the maternal and infant population.
