Job DescriptionThe Medical Mgmt Social Worker 1 provides patient and family assessments in order to design effective treatment programs and follows the patient and family through the continuum of services. The Medical Mgmt Social Worker 1 provides supportive counseling to patients and families, assesses patient discharge needs and coordinates discharge services. Relies on education, experience, professional training and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. Screening of patients/clients for services and intervention is appropriately done as evidenced by appropriate documentation and services. High risk screening indicators are used to appropriately to identify population at risk as evidenced by appropriate documentation. Risk/screening assessment is concluded according to required format and within required time frames as evidenced by appropriate documentation . All activities required to facilitate a smooth and timely discharge o patients from the acute care setting are initiated and completed as evidenced by documentation. All laws, regulations and policies related to social work as they pertain to the position are kept up to dateas evidenced by annual competency checklist. All physician and complex issues are refered to the director and/or manager in a timely manner as evidenced by audits of outlier report Partnership and Collaboration: This will have been satisfactorily performed when... Clinical record contains documentation of appropriate interventions, and plans for ongoing services as evidenced by documentation. Assistance is provided to patient/family with adjustment to illness and medical environment as evidenced by clinical documentation. Actively participates in IDT regarding discharge needs/plans as evidenced by direct observation by Director and/or Manager. Community resources are effectively worked with/coordinated and keeps current on new resources in the community as evidenced by documentation. Interactive communication in planning and implementing of discharge process is conducted. Interacts in a positive, competent and respectful manner with patients, families, caregivers and staff, co-workers, physicians and community agencies as evidenced by no verbal or written complaints. As an expert resource for co-workers on complex cases, collaborates with other social workers to think outside of the box as evidenced by direct observation by Director and/or Manager. Quality: This will have been satisfactorily performed when... Appropriate referrals are made for interventions for patient/family. Community resources are used appropriately as evidenced by documentation in the clinical record. Documentation contains all referrals and clinical interventions related to alcohol/substance abuse, neglect, abuse, domestic violence within clinical record within one working day as evidenced by documentation. The patient advocacy role is demonstrated through communication and documentation as evidenced by documentation. Effectively manages assigned caseload, utilizing strong organizational skills and working independently as evidenced by direct observation and audits by Director and/or Manager. Resources with needed Information are found and understanding of the impact of clinical data occurs. Documentation included related to patient and/or family understanding of barriers related to appropriate care and transition planning as evidenced through clinical record documentation.
