Job DescriptionJob Title: Social Worker - Cardiovascular Intensive Care UnitSocial Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

• This position involves the provision of social work services to patients in the Cardiovascular Intensive Care Unit (CVU) which is located on GP 5 West.
• Responsibilities include coverage of the 17-bed unit.

Responsibilities

• The social worker assigned to this unit is responsible for conducting comprehensive psychosocial assessments of all patients.
• This is a complex position in which there is a need for providing crisis intervention, ongoing therapeutic and grief counseling, supporting of families who are struggling with prognosis and/or end of life decision making with a component of palliative care work.
• The social worker will take a leadership role in family meetings.
• Thorough knowledge of crisis, family and systems theories is required in this interdisciplinary setting.
• The social worker is an integral member of both the Institute of Critical Care Medicine (ICCM) and the Heart Hospital with responsibilities including quality improvement projects, committee meetings and participation beyond the purview of the floor assignments.
• The social worker is expected to identify patterns of need, develop and implement programs and use and monitor quality improvement methods and metrics.
• Coverage will include other ICUs.

Qualifications

• Excellent clinical, collaborative and organizational skills.
• Ability to take lead in coordinating multiple disciplines for complex family meetings and planning
• Knowledge of crisis intervention and end-of-life treatment.
• Ability to coordinate resources within the hospital and community networks for effective management of patient care.
• Inpatient hospital experience.
• LCSW preferred.
• Minimum one year in current position unless currently in Cardiac or Surgery.
