Under the supervision of an LICSW, provide case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. Intervene as necessary preventing over and under utilization of health care resources. Actively participate in discharge planning to assure timely individual patient discharges within the scope of position responsibilities.Qualifications* A Masters Degree in Social Work from an accredited college or university or the equivalent knowledge or experience is required.* Current Massachusetts Social Work license at a minimum of a LCSW is required.* Over three (3) years of related work experience including one (1) year experience in a medically oriented setting is required.* Computer skills are essential.* Certification in Case Management is preferred.* Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.Pay RangeUSD $27.13 - USD $45.60 /Hr.Rotation Schedule Requirementat least one holiday per year
