Critical Tme Intervention (CTI) is a community-based support for the mentally ill population with complex needs. The service is time limited with a phased-out approach. The purpose is to promote community integration and continuity of care. The social worker and client will collaborate to complete person-centered assessment.
- Social worker will meet with existing collaterals and family to monitor and strengthen clients support network.
- Community home visits are required for this role. .
Requirements - LMSW or LCSW issued by the New York State Education Department required.
- (Provisional License holders may also be considered).
- Hospital/Medical experience is preferred but new grads are welcome
- Knowledge of community-based organizations and resources for discharge planning.
- Ability to multi-task and prioritize in a fast-paced, high-volume setting.
- Excellent communication, organizational, and time management skills. PC Proficiency is required. Superb written and oral communication skills.
- Strong crisis intervention and short-term counseling skills
