Job SummaryPerform and document psychosocial risk assessments, as delegated by a licensed Social Worker. Document conversations and interventions performed for discharge planning process, according to hospital policy and procedures. Assist with discharge planning. Provide courteous and prompt response to patient requests/inquiries. Gather and document basic objective data and report findings to the licensed Social Worker and/or Case Manager . Assist/participate in MDTR's. Submit referrals within Ensocare, for DME/HH/SNF/Rehab, etc. Complete PAS assessment for nursing facility placement and enter into designated Utilization Management Contractor portal, within hospital and state guidelines

Obtain choice forms5. Discuss and assist with Advanced Directives6. Participate in MDTR's7. Complete and submit referrals in Ensocare for DME, Home Health, SNF, Med Rehab, etc.8. Arrange transportation assistance to facilitate discharge9. Assist licensed Social Worker and/or Case Manager with discussion for discharge planning, including with patient/family/provider/care team10. Arrange for community resources to facilitate discharge11. Document all interventions/activity in the Medical Record12. Assist with meds-to-beds or arranging for medication at discharge13. Perform work in a cost-effective manner14. Other duties as assigned

Education •Formalized Training (Required) Education: Enrolled in a Bachelor's or Master's level program for Social Work and classified as a senior or fourth yearExperience: Must have completed at least 3 years of a bachelor's level program and be classified as fourth year/senior level
