GENERAL SUMMARY:To provide psychosocial assessment planning, treatment and education for adult and geriatric patients and their families in collaboration with all members of the interdisciplinary group.PRINCIPLE DUTIES AND RESPONSIBILITIES:Provides Medical Social Work Services in the place ofresidence to adult and geriatric populations and their families through complete assessment of the psychosocial needs affecting the patient/family and making appropriate written plans for care.+ Participates in agency and system-wide continuous quality improvement activities to improve performance and patient outcomes.+ Collaborates with leadership to develop and implement policies, procedures, CQI activities, reporting, training and education to ensure organization-wide compliance with legal, ethical, contractual and/or regulatory standards (i.e. TJC, HCFA, OIG, Corporate Compliance, MIOSHA, etc.)+ Develops partnerships with all members of the health care team and exhibits behaviors that displays teamwork, professionalism and respect for diversity.+ Participates in the orientation of new members of the health care team and exhibits behaviors that promote a positive learning experience.+ Maintains a safe and risk-free environment for patients, their families/significant others and other staff.+ Creates a caring experience for both internal and external customers; is courteous and respectful, anticipates and responds to customer needs; and gives high priority to customer satisfaction.+ Demonstrates personal accountability for own continuous growth and development and seeks out appropriate learning opportunities to enhance competency and improve performance.EDUCATION/EXPERIENCE REQUIRED:Master's Degree in Social Work accredited by the Council on Social Work EducationOne (1) year of work experience in health careOne (1) year experience in Hospice preferredCERTIFICATIONS/LICENSURES REQUIRED:Current Michigan Licensure - MSWCurrent BLS certification#LI-VD1Additional Information+ Organization: Henry Ford Jackson Hospital+ Department: HOSPICE RESIDENCE+ Shift: Day Job+ Union Code: Not ApplicableAdditional DetailsThis posting represents the major duties, responsibilities, and authorities of this job, and is not intended to be a complete list of all tasks and functions.
