SCOPE OF ROLE:
The Social worker will be responsible for the overall delivery and coordination of services for Behavioral Health programs. The Social Worker is responsible for interfacing with clinical staff, mental health and medical providers, and relevant community-based organizations to enhance and ensure individuals are in receipt of optimal care and services. The Social Worker will be responsible for the completion psychosocial assessments to assist in permanent stable housing. The Social Worker will be responsible for assessing individuals needs and facilitate appropriate linkage to community-based services. Work within a crisis driven Behavioral Health environment with a knowledge of substance abuse, NYC Housing Application (2010E), trauma and dual diagnosis.Location:

Qualifications: Current licensure in LMSW required from a school of social work fully accredited by the Council on Social Work Education (CSWE) or current licensure in LMHC Knowledge of emotional/behavioral health, mental health, dual-diagnosis Ability to work independently and as part of a multi-disciplinary team, to assist individuals of diverse ethnicities, backgrounds, ages, and sexual preferences Effective written and oral communication skills.PREFERRED QUALIFICATIONS & SKILLS
