JOB SUMMARYResponsible to provide psychotherapy to clients at the Addiction Stabilization Center. Social Worker will assess and evaluate behaviors as well as mental and emotional health providing therapy to stabilize symptoms. Assist the team to establish a referral for a client's continued care upon discharge. ESSENTIAL JOB FUNCTIONS
- Provide individual counseling, advocacy, group therapy, crisis intervention and discharge planning.
- Provide family counseling sessions to assist in understanding and working with the client's recovery.
- Secure appropriate releases of information in accordance with the federal regulations of the Health Insurance Portability and Accountability Act.
- Evaluate, assess and provide case management services.
- Coordinate referrals for appropriate services.
- Ensures that all required documentation is completed on time with quality.
- Use the electronic medical record for all documentation.
- Attend treatment meetings as part of an interdisciplinary team.
- Assist in the development and review of policies and procedures.
- Provide clinical trainings on mutually agreed upon topics relating to mental health to staff at ASC.
- Opportunity to supervise internship practicum
- Informs direct supervisor and/or administrator on call regarding incidents.
- Collaborate with the Psychiatric Nurse Practitioner as needed to address any concerns.
- Abide by all Mission, program and safety policies, procedures and guidelines.
- Attend and participate in meetings and trainings as requested by your supervisor.
- All other duties as requested by your supervisor.
MINIMUM JOB QUALIFICATIONS
- LMSW, LCSW, LMHC or certified Family Therapist.
- (1) year of experience working with substance abuse population preferred.
- Comply with NASW code of ethics.
- Must obtain CPR certification within 90 days of hire and maintain current certification throughout period of employment.
- Intermediate knowledge of Microsoft Word, Excel and Outlook.
- Clean/valid NYS Driver's License.
