TITLE: Social Worker DEPARTMENT: Youth and Community Development ServicesLOCATION: Bedford Stuyvesant, Brooklyn (The Stockton Community School P.S. 297)PROGRAM: Community Schools InitiativeREPORTS TO: Community School DirectorSALARY: $60,000- Compensation based on job experienceSCHEDULE: Monday - Friday, 8 am - 4 pm or 9 am - 5 pm; Some Saturdays or eveningsJOB SUMMARY: To provide at-promise students and families with guidance and support services. The social worker will advocate on their behalf to ensure students and their families gain access to the necessary mental health support services needed to succeed academically. The social worker will work closely with the Community School Team (director, case manager, outreach worker, and tutor) to provide resources that will enhance the Community School.EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS:
- Masters Degree in Social Work OR related field required; LMSW and SIFI preferred
- At least 3 years' experience leading counseling groups, providing clinical services, and/or serving as a case manager for youth and families.
- Experience working in NYC Public Schools and with youth in underserved communities
- Ability to develop alliances with high-risk and difficult-to-engage youth and families
- Excellent facilitation and communication skills
- Strong organizational and computer skills
- Demonstrate the ability to handle multiple tasks
- Outstanding collaborative skills
- Bilingual Spanish/English preferred
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Recruit 100% of students identified as the targeted population (mental health needs students, special education students, students living in transitional housing, and students with attendance issues).
- Conduct weekly group counseling sessions
- Provide confidential, individual counseling to participants
- Provide crisis intervention, short-term issue resolution, and continued follow-up for youth and families
- Maintain records of all individual meetings, baseline assessments, confidential correspondences, and case notes in database and/or a locked file.
- Input and update program database
- Make contact and referrals with other existing community resources that support the development of students and their families
- Responsible for keeping track of targeted participants' progress (academic, social, and emotional) and helping them to address specific issues that might impede their academic progress.
- Develop individualized educational achievement action plans for each student in the program
- Work closely with outreach worker, case manager and tutor to identify targeted student needs to inform student interventions
- Maintain ongoing open communication with school principal, parents, and school-based staff
- Participate in ongoing efforts to advocate for program needs via community coalition building and via administering feedback surveys and program assessments
- Seek partnership opportunities and bring in at least 3 new partners a year to leverage resource to support programs
- Attend Pupil Personnel Team, Individualized Education Program, and/or attendance meetings
- Accompany outreach worker on home visits
- Attend bi-monthly GSS team meetings
- Attend all required department/ agency meetings and /or meetings
- Perform other duties as assigned by the Community School Director
